How In-Home Care Services Work
Once you have made the decision of choosing Trinity Life at Home, we will perform a nurse assessment in the comfort of your own environment.
Trinity Life at Home conducts the assessment in concert with the client, family caregivers, and medical practitioners to get a full understanding of the client’s needs.
We re-evaluate our plans every 90 days or if a change in condition occurs to ensure we have the best care plan in place.
